This form is for printing and placing your order off line, via fax or mail only.     

To shop on-line, go to the ON-LINE STORE

 

1) Complete the Shipping and Billing Information below by typing in the form fields provided.  (You may print the page and hand write the information as well.)

 

2) Print the page by either selecting your printer icon in your browser window or selecting file, then print.  (You can then delete your personal info from the form fields.)

 

3) Go to the On-Line Store.  Select the first item you would like to order.  After clicking on the "order" button, a new page - your "shopping cart" - will open, displaying the item you selected along with the calculated shipping charge. 

 

4) If you would like to add more items, select the "continue shopping" button.  You may get a security notice that you are leaving a secure area, but you have not processed any personal information so there is no need to worry! 

 

5) After you have selected all your items, print the "shopping cart" page by either selecting your printer icon or going to file, then print.  Close the browser window where your shopping cart page was printed.  This deletes your order.
